Rethinking Skin Aging: The Menopause Factor

Skin is a hormonally responsive organ, and the cells that produce collagen and keep skin hydrated and firm respond directly to estrogen.

Most of what we know about skin aging comes from research on UV damage and general collagen decline over time. What's often missing from that picture is a factor specific to women's biology: the hormonal shift around menopause, and the role it plays in how the skin changes. It's not a single event, but a hormonal transition that unfolds over several years. And it accelerates sharply once estrogen settles at its new, lower baseline after menopause.

What happens biologically around menopause

Skin is a hormonally responsive organ, and the cells that produce collagen and keep skin hydrated and firm respond directly to estrogen. As estrogen levels shift through the menopause transition, those cells slow down fast.

The fact is that women lose up to 30% of their skin's collagen in just the first five years following menopause once estrogen has settled at its new, low baseline. And then keep losing a smaller amount each year after that. That's a much faster drop than just aging over time alone would cause. This isn't news but comes from a well-known 1987 study, and later research confirmed it by measuring collagen directly in skin samples. That same research also found that replacing estrogen partly reversed the loss.

The same hormonal drop also makes skin drier, slower to heal, and less elastic. These aren't just cosmetic changes but reflect real changes in how the skin is functioning underneath.

Why this matters beyond skincare

Skin is one of the few parts of the body we can actually see and touch, which makes it a useful early signal for what's happening elsewhere. The same hormonal shift driving skin changes through the menopause transition is also affecting bone density and heart health around the same time. That doesn't mean skin changes cause problems in bones or the heart, but they tend to happen together and for the same underlying reason.

What the evidence actually supports

Since skincare marketing tends to run well ahead of what's been shown in clinical research, here is a breakdown:

Backed by strong evidence:

Tretinoin, a prescription retinoid, has the best evidence. Multiple large studies confirm it reduces wrinkles caused by sun damage. It works by boosting collagen production and slowing collagen breakdown. Other retinoids like retinol also help, just with slightly less research behind them.

Daily sunscreen and vitamin C serums are well proven for sun-related skin damage specifically. This is a different type of aging than the hormonal kind described above, but the two add up together.

Real effect but rarely mentioned:

Hormone therapy (HRT) has a genuine and measurable effect on skin. A 2023 study review covering nearly 1,600 women found real improvements in skin elasticity, thickness, and collagen with hormone therapy, particularly when started close to the onset of menopause. That said, it's not approved or marketed specifically as a skin treatment, and researchers still want bigger, more consistent studies before drawing firm conclusions. For now, better skin is a nice side effect of therapy taken for other reasons and not a reason on its own to start it.

Still an open question:

Collagen supplements are the most debated item here, so let's unpack why. Several earlier studies, involving well over a thousand women in total, found real improvements in skin hydration and elasticity from taking collagen. But a newer 2025 review took a closer look and found something interesting: when researchers split the same set of studies by who funded them and by study quality, the picture changed. Studies not funded by supplement or pharmaceutical companies, and the highest-quality studies overall, showed no significant effect on hydration, elasticity, or wrinkles. While industry-funded and lower-quality studies did. The supplement industry has strongly disputed how that review was conducted. So for now, nobody can say for certain whether collagen supplements work. It's still an open argument in the scientific community, with no settled answer either way.

A closer look at a fast-growing trend:

Estrogen face creams, often estriol-based, have surged in popularity. Several small randomized trials on topical estradiol have found consistent, statistically significant increases in facial collagen and skin thickness. The limitation is that most of these studies are small, and results vary depending on which type of estrogen is used: one trial using a different form, estrone, found no benefit for sun-damaged skin and even an increase in an enzyme linked to collagen breakdown. The evidence doesn't yet cover all formulations equally, and larger studies are still needed.

The bigger picture

Skin changes through the menopause transition aren't just about looks but a visible sign of a bigger hormonal shift that's also affecting bone, metabolism, and heart health. What we see in the skin is one piece of a much larger picture, and it's best understood alongside everything else, not on its own.
